Workplace Injury Routing Guide

Emergency First

Call 911 or route to emergency care for severe symptoms, uncontrolled bleeding, loss of consciousness, major trauma, severe eye injury, severe chemical exposure, chest pain, stroke symptoms, or any life-threatening concern.

Occupational Medicine Evaluation

Non-emergency sprains, strains, minor cuts, minor burns, and follow-up restrictions may be appropriate for occupational medicine.
Document the incident, supervisor contact, job duties, and modified-duty options.
Give the employee clear instructions for evaluation and follow-up.

Return-To-Work Notes

Track restrictions, follow-up date, modified duty availability, and who receives work-status documentation.
