Diabetes and DOT Physicals: Navigating FMCSA Rules for Insulin and Non-Insulin Drivers

Managing diabetes while maintaining a commercial driver's license requires close attention to FMCSA medical guidelines. Diabetes is a safety-sensitive condition because severe hypoglycemia can cause confusion, loss of consciousness, or seizures, creating dangerous driving scenarios.

Fortunately, having diabetes does not automatically disqualify you from driving a Commercial Motor Vehicle (CMV). The path to certification depends on whether your condition is managed with insulin or non-insulin treatments.

Non-Insulin Managed Diabetes

If your diabetes is managed through diet, exercise, or oral medications (such as Metformin), your certification process is straightforward:

  • You must demonstrate that your blood sugar is under control.
  • Bringing a recent HbA1c laboratory report (completed within the last 90 days) showing stable control is highly recommended.
  • Non-insulin diabetics can be certified for the standard 24-month (2-year) period, depending on overall health.

Insulin-Treated Diabetes Mellitus (ITDM) Guidelines

In 2018, the FMCSA replaced the old individual exemption application process with the Insulin-Treated Diabetes Mellitus (ITDM) Assessment Program. Under this program:

  • Drivers must be evaluated by their treating clinician (TC) who manages their insulin therapy.
  • The TC must complete the Insulin-Treated Diabetes Mellitus Assessment Form, MCSA-5870.
  • The form confirms the driver has a stable insulin regimen, is compliant with glucose self-monitoring, and has not had severe hypoglycemic episodes.
  • You must bring the signed Form MCSA-5870 to your DOT exam.
  • The certified medical examiner can then qualify you for a maximum of 12 months (1 year).
