Passing the blood pressure portion of your DOT physical is crucial to keeping your Commercial Driver's License (CDL). The Federal Motor Carrier Safety Administration (FMCSA) enforces strict blood pressure requirements to ensure safety-sensitive drivers do not suffer sudden cardiac events while on the road.

FMCSA Blood Pressure Classification and Certification Tiers

Here are the exact blood pressure readings and corresponding certification periods:

1. Normal Blood Pressure: Under 140/90 mmHg

  • Certification Period: Up to 24 months (2 years).
  • Notes: This is the standard qualification period. If your BP is below 140/90, and you meet all other physical standards, you qualify for the maximum 2-year medical card.

2. Stage 1 Hypertension: 140-159 / 90-99 mmHg

  • Certification Period: 12 months (1 year).
  • Notes: If your BP is in this range on the day of the exam, you can only receive a 1-year certificate. You will need to monitor your blood pressure and renew your card annually.

3. Stage 2 Hypertension: 160-179 / 100-109 mmHg

  • Certification Period: One-time 3-month temporary certificate.
  • Notes: This is a warning tier. The examiner can issue a single 3-month certificate to allow you to seek medical treatment and lower your blood pressure. Once lowered under 140/90, you can be certified for the remainder of a 1-year period from the original exam date.

4. Stage 3 Hypertension: 180/110 mmHg or Higher

  • Certification Period: Immediate Disqualification.
  • Notes: A reading in this range represents severe hypertension and is an immediate safety risk. You cannot be certified until your blood pressure is treated and successfully lowered to 139/89 or below.
