Failed Your DOT Physical? What to Do Next to Get Back on the Road

Learn the difference between a temporary pending status and a medical disqualification, and how to appeal or fix it.

Failing a DOT physical can be stressful for commercial drivers whose livelihoods depend on keeping their medical cards. However, in many cases, a 'fail' is not permanent. It is often a temporary delay caused by lack of documentation or easily treatable metrics.

Determination Pending vs. Disqualification

It is important to understand the two outcomes when a driver does not meet the standard:

1. Determination Pending (Temporary Hold)

  • What it means: The examiner has not failed you, but needs more information. They have up to 45 days to keep your exam open while you gather records.
  • Common causes: High blood pressure on the day of the exam, missing CPAP compliance reports, or missing cardiologist clearances.
  • Fix: Provide the missing records or complete treatment within 45 days. You can continue driving under your current card until it expires.

2. Medical Disqualification (Failure to Qualify)

  • What it means: You do not meet the minimum health requirements to operate a CMV safely.
  • Common causes: Stage 3 hypertension, uncontrolled seizure disorders, severe untreated sleep apnea, or severe vision/hearing loss.
  • Fix: Seek medical treatment. Once your condition is controlled (e.g., blood pressure lowered or CPAP compliant), you can undergo a new exam and be certified.
