Arriving unprepared for your DOT physical can result in a pending determination, requiring a follow-up visit and delaying your medical card. To help you pass on your first try, we have compiled the definitive list of what you must bring to the clinic.

Every Driver Must Bring:

  • Commercial Driver's License (CDL): Or a valid government-issued photo ID.
  • Medication List: A complete list of all prescriptions, showing dosages, schedules, and the name of the prescribing physician.
  • HSA/FSA or Payment Method: Card or employer authorization form.

Bring These If They Apply to You:

  • Vision correction: Wear your glasses or contacts to the appointment.
  • Hearing aids: Wear them to the exam.
  • CPAP Therapy: A printed 90-day compliance report from your machine showing usage of 4+ hours/night on 70%+ of nights.
  • Insulin-Treated Diabetes: Signed Form MCSA-5870 completed by your treating endocrinologist, plus recent HbA1c labs.
  • Heart Conditions: Bring a recent clearance letter from your cardiologist (within 12 months) and recent stress test results.
