CDL Medical Certificate Florida: How Long Is It Valid?

Your CDL medical certificate is one of the most time-sensitive documents in your wallet. Let it expire and you are not just out of compliance. You are off the road immediately. Here is exactly how long your certificate is valid in Florida and what affects that timeline.

The Standard: 24 Months

Most commercial drivers who pass their DOT physical without restrictions receive a medical certificate valid for 24 months. This is the FMCSA maximum. No Medical Examiner can issue a certificate valid longer than two years regardless of how healthy you are.

Mark the expiration date the day you receive your card. Set a reminder 60 days out. Do not wait until the week before. Scheduling delays, a borderline blood pressure reading, or a required specialist letter can push your renewal past the expiration date and take you off the road.

Conditions That Shorten Your Certificate

This is where most drivers get caught off guard. Certain medical conditions automatically reduce your certification period regardless of how well controlled they are.

High blood pressure:

Blood pressure is the most common reason Florida CDL drivers receive a shortened certificate.

  • Stage 1 hypertension (140-159/90-99): certified for 12 months, must recheck at renewal
  • Stage 2 hypertension (160-179/100-109): one-time 3-month certificate, must demonstrate treatment and control before full certification
  • Stage 3 hypertension (180+/110+): disqualified until controlled

If your blood pressure is borderline, check it several times in the days before your exam. Stress, caffeine, and even driving to the appointment can spike your reading temporarily. Arrive early, sit quietly, and breathe before the cuff goes on.

Insulin-treated diabetes:

Insulin-treated diabetes mellitus (ITDM) previously disqualified drivers from interstate commerce. FMCSA changed this rule and ITDM drivers can now obtain exemptions. However certification is annual and requires:

  • Treating endocrinologist documentation
  • Three months of stable glucose logs
  • No severe hypoglycemic episodes in the prior 12 months
  • Completed FMCSA ITDM exemption application

If you are managing diabetes with insulin, bring full documentation to your appointment. Do not show up without it.

Sleep apnea:

Diagnosed obstructive sleep apnea requires annual certification. You must demonstrate PAP therapy compliance, typically 70% usage over 30 days documented by your device. Bring your compliance report from your sleep therapy provider.

Undiagnosed but high-risk drivers may receive a temporary certificate pending a sleep study. Do not ignore a referral. It will not go away and delays your full certification.

Epilepsy and seizure history:

Generally disqualifying for interstate commerce unless seizure-free for a defined period off medication. Requirements vary. Bring full neurology records if you have any seizure history.

Vision and hearing limitations:

Corrected vision to 20/40 in each eye satisfies the standard. Color recognition and peripheral vision are also tested. Hearing aids are permitted and effective. Bring yours if you use one.

What Happens If My CDL Medical Certificate Expires in Florida?

If your medical certificate expires your CDL downgrades automatically. In Florida, FLHSMV receives updated medical information from the FMCSA Clearinghouse. Once your certificate lapses:

  • Your CDL is downgraded to a regular Class E license
  • You cannot legally operate a CMV
  • Driving with an expired medical certificate is a federal violation
  • Carriers face liability for allowing expired drivers to operate

Reinstatement requires a new DOT physical, updated FMCSA registry entry, and a visit to the FLHSMV to restore your CDL class. The process takes time. Do not let it get to this point.
